Position: Clinical/DUR Pharmacist

Location: Centurion, Gauteng

Position Purpose: To ensure accurate and timely processing of chronic medicine reviews in alignment with client specifications, while providing expert clinical input to optimize medication management and patient outcomes. To assist with operational activities within the PHA Medicine Management space.

Experience: Minimum of 1 year experience with capturing medicines in a DUR/PHB environment with

  • years previous working experience within a Managed Care or Pharmaceutical environment (Essential).

Qualifications: Bachelor of Pharmacy. Registration with SAPC.

Key Performance Areas

Chronic Medicine Review

  • Conduct comprehensive reviews of chronic medication regimens for accuracy, appropriateness, and compliance with client protocols.
  • Identify potential drug interactions, duplications, and
  • Ensure documentation and reporting meet regulatory and client standards.

Clinical Input

  • Provide evidence-based recommendations for therapy optimization in chronic disease
  • Collaborate with healthcare providers to address clinical queries and improve treatment
  • Stay updated on clinical guidelines, formulary changes, and best practices in chronic

Quality Assurance

  • Maintain high standards of accuracy and consistency in medication
  • Participate in audits and implement corrective actions where

Stakeholder Engagement

  • Communicate effectively with clients, prescribers, and internal teams to resolve issues and enhance service delivery.
  • Offer training and support on chronic medicine protocols when required.
