Reducing the adverse effects of chronic conditions and reliance on medicine

The Private Health Administrators™ (PHA) focus on the management of chronic conditions extends beyond standard compliance monitoring, to the facilitation of lifestyle changes through an individually tailored lifestyle management programme. This unique range of programmes uses the common thread of nutrition, exercise/movement and psychology to positively alter the health status of members who are affected by a chronic illness or debilitating ailment.

The PHA disease risk and medicine adherence management programme combines the practices of the member’s healthcare provider, biokineticists, nutritionists, education and the PHA Nurse Administrator to work with the member on a tailored programme designed to modify their experience of their illness. These programmes are either subsidised by the medical scheme or the employer company.

The programme reaches beyond simple risk assessments and seeks to assist members with reducing the adverse effects of chronic conditions, monitor and educate on the importance of treatment adherence and minimise unnecessary hospital admissions.

Potential participants are identified through screening programmes or claims data and encouraged to join the Disease Risk Management Programme.

PHA uses various exercise and diet programmes, Registered Dieticians, and Registered Biokineticists together with a dedicated Registered Nurse to encourage members and measure progress.
